FOCUSED FUNDING

 In response to current economic conditions, throughout 2010 United Way Fox Cities' Focused Funding Grants will be designated to agencies or programs providing basic needs services (food, clothing, housing, mental / physical / dental care and services directed at preventing homelessness such as rental and utility assistance).  

 

Agencies and programs that have received the maximum of three grants in previous years and would otherwise be ineligible for Focused Funding are eligible to apply.  

 

CRITERIA FOR AGENCIES SEEKING FINANCIAL SUPPORT

  • The governing agency must be tax exempt and the proposed program address the basic needs of the people of Outagamie, northern Winnebago or northern Calumet counties.
  • The governing agency must have written bylaws and a community-based board of directors or equivalent.
  • Intended outcomes must be stated in the letter of intent.
  • Eligible agencies must document an increase in demand for services and a revenue shortfall.

United Way Fox Cities' Community Readiness Committee will review applications on a quarterly basis.  The committee is composed of the volunteer leadership of the Community Impact Council and representatives from United Way's four Impact Areas and the Outcomes Advisory Committee.

 

Fourteen (14) copies (three hole punched) including the original of the completed application must be submitted to United Way Fox Cities, 1455 Midway Road, Menasha, WI 54952 by 4:30 p.m. on the stated deadline date.  When submitting copies of the application, do not bind them in any fashion or place materials in folders or binders.  The use of paper clips is acceptable.  Please note that only one copy of some attachments is required.

APPLICATION FORMS

 

The application will consist of the following forms:

  • Actual income and expense statement for the past fiscal year
  • Income and expense statement for the current fiscal year

Upon receipt United Way staff will review the application and may contact the applicant with questions about the request.  A time may be scheduled for applicants to meet in person or by a phone conference with the Community Readiness Committee to discuss the request.  Those dates are listed below along with dates the grant recommendations will be presented to and acted on by the Community Impact Council and the Board of Directors.  Applicants will receive written notification of the decision whether or not funding is awarded.

  • All grant recipients are expected to submit a written evaluation six months after receiving funding.

Due to limited funds available for Focused Funding Grants, an agency may be awarded up to $10,000.  The agency may return later in the year if the need for additional funding is clearly demonstrated and funds remain available for distribution however, second requests may be given a lower priority when considered with other, first time requests.

Grant Approvals

  • Grants awarded to tax-exempt, 501(c)(3) organizations are approved by the Community Impact Council.
  • Grants awarded to tax-exempt, non 501(c)(3) organizations must be approved by the United Way Fox Cities Board of Directors.

When submitting the application, please include the following items:

  • Common Grant Application Cover Sheet (Part A)
  • The completed letter of intent (no more than three pages)
  • The Project Budget Form if applying for a specific program
  • The organization's income and expense statement for the past fiscal year
  • The organization's income and expense statement for the current fiscal year.

Additional Attachments:

  • One copy of the organization's most recent audited financial statement or financial review if not already on file with United Way Fox Cities
  • One copy of the IRS Federal tax exempt determination letter, if not already on file with United Way Fox Cities.
